Applications can be emailed to Dr. Heather Hamlin (hjhamlin@ufl.edu) or mailed as hard copies to the address below.
Letters of recommendation/support should be mailed with the FERPA form by the person writing the letter. If the person writing the letter prefers to email their letter of recommendation, they should retain the FERPA form for their own records.
